PHPV – Calcified subluxated cataract


 

Case of persistent hyperplastic primary vitreous, eye shorter than normal, thickened sclera, and a luxated cataract in resorption with vitreous in the anterior chamber. The loose cataract was causing chronic anterior uveitis and pain, and at different visits would be half in the anterior chamber and half in the vitreous cavity. As the video shows, the initial anterior approach showed a heavy, calcified cataract that would not let itself be emulsified or chopped, and would easily end up in the vitreous cavity. The persistent hyaloid artery attachment was vitrectomized. The darkened retina had widespread atrophy.
